Fees and payment (updated 10/2019)
I am in network with BCBS, Pacific Source, Moda and Cigna. I do not take medicaid/medicare at this time. Your insurance will be billed with any deductible or co payment amounts due charged to your card on file.
I also accept private pay, providing further discretion. For private pay services, payment is due at time of service.
Cancellation policy is as follows: Please give a 48 hours notice. Cancellations must be managed online using your email confirmation. You can reschedule or cancel your appointments online using the booking link. You will be sent a verification code to your email on file.
No-shows or missed appointments without 24 hour notice: $100 fee before you will be allowed to reschedule. Exceptions for true emergencies (family and medical emergencies).
This is not meant to be punitive, with less than 24 hour notice, the appointment time is not able to be filled by another patient and 24 hours before your appointment, time is spent preparing for your appointment. By scheduling an appointment you are consenting to this policy.

It is important to know what your health benefits are and what the out-of-pocket costs will be based on your specific plan. If you would like me to bill your insurance, I recommend that you contact your company and ask the following:
What is my approach to medications?
My prescribing practices include using the minimal effective dose of medications, reducing the number of medications one takes and limiting the use of controlled substances. I aim to use minimal amounts of medication, for the shortest amount of time possible, with a focus on safely discontinuing and tapering medications when able.
I will refill medications within 48 business hours of request via the medication refill link on this page. I will contact you for any issues or if you need to come in for an appointment prior to refill.
After 10 minutes, appointments will be cancelled, or in case of an emergency, appointment time will be reduced without refund.
Communication and phone calls
My preferred mode of communication is by email first email@example.com, and then by phone 503-606-6355. I will return currentpatient emails within 48 business hours. All emails are HIPPA compliant. I have a patient portal you can have access to for more security.
I will return phone calls for current patients with routine medication or scheduling questions under 5 mins are, as a courtesy, free. For calls greater than this, a rate of $75 for every 15 mins may be charged on a case by case basis.
I only prescribe controlled substances if I am your only prescriber for these medications. You must be seen at least every every 30 days if dose adjustments are needed and 90 days once stable.
I do not provide the following services
In the case of emergency, such as thoughts of harming yourself or others, please call 911 immediately or go to the nearest emergency room. You may also call the national suicide prevention lifeline at 1-800-273-TALK (8255), which is a confidential hotline staffed by trained mental health counselors 24 hours per day, 7 days per week.
